Bleeding after hospital discharge following acute coronary syndrome: Incidence, types, timing, and predictors

Ismail N, et al. - Researchers used the Clinical Practice Research Datalink joined to Hospital Episode Statistics, to evaluate the incidence, types, timing, and predictors of bleeding complications after hospital discharge post-acute coronary syndrome. This study included 27,660 patients who survived to hospital discharge. Within the first 12 months following hospital discharge, the incidence of bleeding was estimated to be 162/1000 person-years (95% CI, 157–167/1000 person-years). The most common type of first bleeding event was bruising (949 bleeds [26%]). The next common type was gastrointestinal bleed (705 bleeds [20%]), whereas intracranial bleed was relatively rare (81 bleeds [2%]). History of bleeding complication, oral anticoagulant prescription, history of peripheral vascular disease, chronic obstructive pulmonary disease, and advanced age (> 80 years), were identified as significant predictors of bleeding after discharge. Overall, a common incidence of bleeding complications following hospital discharge for acute coronary syndrome was reported. Different baseline features, which differed by anatomic site of the bleed, were seen in patients who had these bleeding events. When deciding on favorable combination and duration of secondary antithrombotic therapy, these features could be of help.
