Balloon-expandable vs self-expanding transcatheter aortic valve replacement: A propensity-matched comparison from the France-TAVI registry

Van Belle E, Vincent F, Labreuche J, et al. - Researchers sought to compare balloon-expandable (BE) with self-expanding (SE) transcatheter heart valve (THV) on individual endpoints after transcatheter aortic valve replacement using data of 12,141 patients undergoing BE-THV (Edwards, n = 8,038) or SE-THV (Medtronic, n = 4,103) for native aortic stenosis from the FRANCE-TAVI nationwide registry. Using propensity-score (25 clinical, anatomical and procedural variables) and by date of the procedure (within 3 months), they matched 3,910 patients treated with BE-THV with 3,910 patients treated with SE-THV. Outcomes revealed a higher risk of paravalvular regurgitation and higher in-hospital and 2-year mortality in correlation to the use of SE-THV vs BE-THV. Data gained strongly support the necessity for a randomized trial adequately powered to compare head-to-head the latest generation of SE and BE-THV.
