Association of posttreatment lymphopenia and elevated neutrophil-to-lymphocyte ratio with poor clinical outcomes in patients with human papillomavirus–negative oropharyngeal cancers

JAMA Otolaryngology—Head & Neck Surgery May 08, 2019

Lin AJ, et al. - In this cohort study of 108 patients having treatment for oropharyngeal cancer (OPC), researchers tested the impact of postradiotherapy lymphopenia and elevated neutrophils to lymphocytes (NLR) on clinical results. In patients with human papillomavirus-negative OPC, lymphopenia and NLR as early as 3 months following treatment start can serve as biomarkers of clinical results. From adjuvant treatment intensification or closer surveillance, these subjects can benefit. For this study, 29.2% had grades 3 to 4 lymphopenia 3 months after start of radiotherapy, which was correlated with worse recurrence-free and overall survival. In order to identify patients at risk of recurrence, postradiotherapy immunosuppression may be used.
