Association between estimated GFR and incident hypoglycemia during hospitalization

Khanimov I, Zingerman B, Korzetz A, et al. - Findings demonstrate an association of decreased estimated glomerular filtration rate (eGFR) with elevated risk of hypoglycemia in non-critically-ill patients hospitalized in internal medicine departments. Irrespective of diabetes mellitus (DM) status, glucose monitoring should be performed in all inpatients with CKD.

  • In this retrospective cohort study, 39,316 patients (mean age 68.0 ± 18.0 years, 49.3% males, 25.9% with DM, eGFR 69.5 ± 24.9 mL/min/1.73 m 2 ) discharged from internal medicine units between 2010 and 2013 were analyzed.

  • Occurrence of at least one hypoglycemic event was seen in 6.5% of participants.

  • An inverse relation of eGFR with incident hypoglycemia (OR 0.988) was revealed by logistic regression modelling, and outcomes were similar for patients with and without DM.

  • A negative correlation of estimated GFR with admission CRP levels was found for patients with and without DM.

  • There was a positive correlation between estimated GFR and admission serum albumin levels for both patients with and without DM.
