Assessment of the German and Italian Stress Cardiomyopathy score for risk stratification for in-hospital complications in patients with Takotsubo syndrome

Santoro F, Gil IJN, Stiermaier T, et al. - Through a multicenter prognostic study of 1,007 individuals from the German and Italian Stress Cardiomyopathy (GEIST) registry and 946 patients from the Spanish Registry for Takotsubo Cardiomyopathy, researchers created a simple clinical score to forecast risk of in-hospital complications among individuals with Takotsubo syndrome (TTS). The GEIST prognosis score was created by granting 20 points each for male sex and history of neurologic disorders and 30 for right ventricular involvement and then subtracting the value in percent of left ventricular ejection fraction. There were 316 participants seperated into three risk categories by low risk (n = 342), intermediate risk (n = 342), and high risk (n = 114) of complications using this score. The noted in-hospital complication rates were 12.7%, 23.4% and 58.8% for low-risk, intermediate-risk, and high-risk individuals, respectively. Following 2.6 years of follow-up, individuals with in-hospital complications vs those without complications had significantly greater rates of mortality. In early risk stratification for TTS, the GEIST prognostic score may be beneficial. High-risk individuals with TTS may need care in an intensive unit while low-risk individuals with TTS could be discharged within a few days. In-hospital complications in individuals with TTS a higher long-term mortality risk may be related.
