Assessment of rapid response teams at top-performing hospitals for in-hospital cardiac arrest

Dukes K, et al. - Via performing a qualitative study of data collected from interviews with 158 key stakeholders at 9 Get With The Guidelines–Resuscitation hospitals, experts assessed variations in design and implementation of rapid response teams (RRTs) at top-performing and non–top-performing sites for survival of in-hospital cardiac arrest (IHCA)—which was known to be correlated with hospital performance on IHCA incidence. Variations in RRTs at top-performing and non–top-performing sites were discovered in four domains—team design and composition, RRT engagement in surveillance of at-risk individuals, empowerment of bedside nurses to begin the RRT, and collaboration with bedside nurses during and following a rapid response. RRTs were typically staffed with dedicated team members without competing clinical duties who gave expertise to bedside nurses for the management of individuals who were at risk for deterioration and collaborated with nurses during and following a rapid response, at top-performing hospitals. Bedside nurses were authorized to begin RRTs on the basis of their judgment and experience. RRT members at non–top-performing hospitals, on the other hand, had competing clinical duties and were commonly less engaged with bedside nurses. Nurses at non–top-performing hospitals listed matters about potential consequences from beginning the RRT. Hence, top-performing hospitals highlighted RRTs with dedicated staff without competing clinical duties, that work collaboratively with bedside nurses, and that can be initiated without fear of reprisal. Moreover, different insights into RRTs at hospitals with better IHCA outcomes were suggested by the results.
