Anatomic risk factor for meniscal lesion in association with ACL rupture

Gaillard R, et al. - Through a retrospective review of 412 patients who underwent acute anterior cruciate ligament (ACL) reconstruction in the department from October 2012 to February 2015, researchers evaluated anatomic risk factors for meniscal lesions in regards with acute ACL rupture. A medial meniscal tear and a lateral meniscal tear was noted in 160 (119 males/48 females) and 100 (80 males/20 females) patients. From injury to surgery, the mean estimated time was 3 months. For all measurements, the average intra-class correlations was 0.87 suggesting good reliability. In patients with a medial meniscal tear, the medial femoral condyle was observed to be significantly longer in comparison with the medial tibial plateau in the sagittal plane while the lateral femoral condyle was discovered to be significantly longer in contrast with the lateral tibial plateau in the sagittal plane in patients with a lateral meniscal tear. A less convex lateral tibial plateau was statistically associated with a greater risk of the lateral meniscal tear. Hence, a greater anteroposterior length of the medial/lateral femoral condyle in regard to the medial/lateral tibial plateau was correlated with an increased risk of meniscal lesions in relation to acute ACL rupture. Moreover, in the male population, the lateral compartment seemed to be the most at risk.
