Addition of tumor infiltration depth and extranodal extension improves the prognostic value of the pathological TNM classification for early-stage oral squamous cell carcinoma

Boeve K, Melchers LJ, Schuuring E, et al. - In a retrospective study 211 first primary pT1–T2 oral squamous cell carcinoma patients, with surgical resection as primary treatment, were evaluated by the researchers in order to discover the potential influence of the variations in the 8th edition pTNM classification on the prediction and treatment approach for oral squamous cell carcinoma in a well-defined series of pT1–T2 patients with long-term follow-up. One hundred and seventy-three and 38 patients underwent neck dissection and had frequent clinical neck evaluations, respectively. With the T classification and the N classification, classification according to the 8th edition criteria led to 36% and 16% total upstaging, respectively. In comparison with T2-staged patients, T3-restaged patients (n = 30, 14%) had lower 5-year disease-specific survival rates. In another seven patients, postoperative (chemo)radiotherapy could have been acknowledged based on the 8th edition criteria. Therefore, the addition of tumor infiltration depth and extranodal extension in the 8th TNM classification resulted in the recognition of oral squamous cell carcinoma patients with a worse prognosis who might profit from an enhanced postoperative treatment strategy.
