Acute kidney injury and renal tubular damage in children with type 1 diabetes mellitus onset

Marzuillo P, Iafusco D, Zanfardino A, et al. - Researchers undertook this prospective analysis to assess acute kidney injury (AKI) and renal tubular damage (RTD) prevalence, as well as their rate and timing of recovery in a pediatric population with type 1 diabetes mellitus (T1DM) onset. They observed 185 children after 14 days from T1DM onset. A follow-up was performed 30 and 60 days later in patients who did not recover from AKI/RTD. For diabetic ketoacidosis (DKA) and AKI, a prevalence of 51.4% and 43.8%, respectively, was estimated. In T1DM patients with and without DKA, the estimated prevalence of AKI was 65.2% and 21.1%, respectively. In 136/185(73.5%) patients, RTD was identified. RTD was present in all patients with DKA or AKI. Overall, findings revealed that AKI and/or RTD may be experienced by most patients with T1DM onset, especially if presenting with DKA. Normalization of physiological and biochemical parameters of AKI/RTD was seen over time in all patients.
