A systematic review of the risk of motor vehicle collision in patients with syncope

Chee JN, Simpson C, Sheldon RS, et al. - By analyzing 11 relevant studies of moderate quality with 42,972 participants, researchers assessed the risk of motor vehicle collisions (MVC) for patients with a history of syncope. A systematic exploration of MEDLINE (1946 – 2019) as well as CINAHL, Embase, Psychinfo and the Transportation Research Information Documentation (1806 – 2017) was performed to identify articles about MVC and drivers with vasovagal syncope (VVS), arrhythmic syncope, or syncope not yet diagnosed. Relative to general populations of Canada, the USA, and the UK, lower prospective MVC risk was reported for VVS, and a higher risk was noted for arrhythmic syncope. Overall, VVS patients seemed to carry a very low risk for MVCs, this observation lends support to current guidelines which do not advocate driving suspension for these patients in most cases. Arrhythmic syncope seemed to be related to non-trivial risk although there is too limited data for other forms of syncope to draw definitive conclusions.
