A retrospective cross-sectional study of type 2 diabetes overtreatment in patients admitted to the geriatric ward

Wojszel ZB, et al. - Given that individualization of glycemic control targets in older patients should be done based on functional status and comorbidities, researchers undertook this retrospective cross-sectional analysis to investigate high-risk patients who had evidence of tight glycemic control and therefore at risk of serious hypoglycemia. Hospital records of type 2 diabetes patients admitted to the geriatric ward taking diabetes medications were analyzed. Hemoglobin A1C (HbA1C) ≤ 7.0% [53 mmol/mol] was considered as the primary outcome measure. In older patients admitted to the geriatric ward, a very high rate of tight glycemic control was observed; higher HbA1C targets were recommended for these people. This suggests that diabetes overtreatment in this group is highly likely, associated with a high risk of recurrent hypoglycemia. This is all the more likely since most of them took medications recognized to induce hypoglycemia. The necessity of giving more attention to particular difficulties in diabetes treatment in older people was emphasized, particularly in those experiencing many geriatric syndromes and diseases worsening their prognosis.
