A classification of chronic rhinosinusitis with nasal polyps based on structured histopathology

Brescia G, Alessandrini L, Giacomelli L, et al. - A cohort of 135 individuals with chronic rhinosinusitis with nasal polyps (CRSwNP) was evaluated, based on clinicopathological characteristics, ie, allergic fungal rhinosinusitis (n = 17); non-steroidal anti-inflammatory drug-exacerbated respiratory disease (n = 19); intrinsic asthma (n = 18); extrinsic asthma (n = 21); allergy (n = 21); histologically eosinophilic (n = 22); and histologically non-eosinophilic (n = 17) in order to ascertain whether structured histopathology could be utilize to classify CRSwNP in homogeneous histological clusters. On discriminant analysis, the global error rate was 1.48%, and the stratified error rates for clusters 1, 2, 3 and 4, were 4.34%, 0%, 0%, and 0%, respectively. Cluster 1 was classified via infrequent fibrosis (< 4.5% of cases). Cluster 2 particularly featured neutrophil infiltration, hyperplastic/papillary changes, and fibrosis in 100%, 70%, and, 65% of cases, respectively. In 100% of cases, cluster 3 exhibited fibrosis. Cluster 4 exhibited hyperplastic/papillary changes and fibrosis in 100% and in 92% of cases, respectively. Hence, this study exhibits that among CRSwNP individuals, cluster analysis could recognize various histotypes. The next step would be to examine, in a larger cohort, the clinical (eg prognostic) indications of recognizing such homogeneous clusters of individuals with CRSwNP based on their structured histopathology.
