A simple screening tool to predict outcomes in older adults undergoing emergency general surgery

Zattoni D, et al. - In this prospective observational study between December 2015 and May 2016, 110 individuals of age 70 years and older were analyzed to study the utility of the Flemish version of the Triage Risk Screening Tool (fTRST) to assess precisely, the frailty in an emergency setting. They noted 30-day major complications (Clavien-Dindo Classification 3–5) in 28.2% of cases. They found the highest association of fTRST with major complications. A fTRST score of 2 or higher with a sensitivity of 96% was observed when risk factors for overall 90-day mortality were assessed. They recorded more than twice the average length of hospital stay as long in the group with a fTRST score of 2 or greater (15.2 days) as compared to those with a score less than 2. Hence, they preferred fTRST as an effective tool to predict the postoperative course, to determine care goals, and also to plan for involvement of a dedicated geriatric care team.
