Predicting the risk of apparent treatment resistant hypertension: A longitudinal, cohort study in an urban hypertension referral clinic

Buhnerkempe MG, et al. - In this retrospective, cohort study, researchers used commonly collected clinical data to develop risk prediction models for apparent treatment-resistant hypertension (aTRH), a pervasive clinical problem that is associated with higher prevalence of secondary hypertension, greater risk for adverse pressure-related clinical outcomes, and influences diagnostic and therapeutic decision-making). Participants included 745 patients, predominately African American (86%) and female (65%), with uncontrolled blood pressure. A novel clinical score was developed that enabled good discrimination between those who will and will not develop aTRH, especially amongst those without existing diuretic prescriptions. The investigators reported that, regardless of baseline diuretic treatment status, a low risk score had very high negative predictive value.
