Nondipping pattern on 24-h ambulatory blood pressure monitoring is associated with left ventricular hypertrophy in chronic kidney disease

Jaques DA, et al. - This cross-sectional study investigated the potential determinants of left ventricular hypertrophy (LVH) and mass index (LVMI) in 69 adults with stage 3b-5, nondialysis chronic kidney disease (CKD) who had ambulatory blood pressure monitoring (ABPM) and transthoracic echocardiography performed simultaneously. A stepwise, backward, multivariate, linear and logistic regression was used to evaluate the determinants of LVH and LVMI. LVH was present in 31.8% of patients, and systolic and mean nondipping status was associated with increased LVMI. Results showed a high incidence of LVH among nonrenal replacement therapy CKD patients. Further, the nondipping phenomenon on ABPM was found to be associated independently with LVH and increased LVMI. Estimated glomerular filtration rate of less than 30 mL/min/1.72 m2 and hypertension were associated with LVH and elevated LVMI.
