Long-term risk of sudden cardiac death in hypertrophic cardiomyopathy: A cardiac magnetic resonance outcome study

Greulich S, Seitz A, Herter D, et al. - Researchers assessed the prognostic worth of late gadolinium enhancement (LGE) cardiac magnetic resonance (CMR) for sudden cardiac death (SCD) risk stratification in hypertrophic cardiomyopathy (HCM) vs the European Society of Cardiology (ESC) SCD risk score and traditional risk factors in an >10-year follow-up. This analysis enrolled 220 patients with HCM and LGE-CMR. Following a median observation span of 10.4 years, 203 patients (median age 58 years, 61% male) afforded follow-up data. The presence of LGE was identified in 70% of patients with a median LGE amount of 1.6%, the median ESC 5-year SCD risk score was estimated to be 1.84. According to findings, effective 10-year SCD risk stratification beyond established risk factors was conferred by LGE-CMR in HCM patients. Addition of LGE amount to established risk models might be done to enhance its discriminatory power. In particular, patients with > 5% LGE deserve careful monitoring and might represent suitable candidates for primary prophylactic implantable cardioverter-defibrillator during the clinical long-term course.
