Natural history and profile of selective cytokines in patients of acute pancreatitis with acute kidney injury

Prasada R, Muktesh G, Samanta J, et al. - Researchers analyzed the natural course of patients having acute pancreatitis (AP) with acute kidney injury (AKI) as well as their cytokine profile. Overall 97 patients participated. At presentation as well as at 72 h, measurements of levels of TNFα, IL-6, IL-10, IL-8 and IL-1β were obtained in patients who encountered AKI. In the whole sample, AKI development occurred in 16.4% patients. These patients had a death rate of 25%. The AKI group was identified to have significantly higher levels of IL-6 and IL-8. AKI was independently predicted by IL-8 levels at baseline, as revealed in multivariate analysis. For the prediction of AKI, a sensitivity and a specificity of 87.5% and 59.2% could be yielded, respectively, by IL-8 levels at baseline, at a cut-off of 105 pg/ml, as well as an area under the curve of 0.744 was generated. Overall, findings revealed poor prognosis in AP patients encountering AKI, as well as showed the ability of IL-8 levels in predicting AKI in patients with AP.
