Prevalence of permanent pacemaker implantation after conventional aortic valve replacement—A propensity-matched analysis in patients with a bicuspid or tricuspid aortic valve: A benchmark for transcatheter aortic valve replacement

Haunschild J, Misfeld M, Schroeter T, et al. - Researchers sought to present benchmark data—from a high-volume centre [with a competitive transcatheter aortic valve replacement (TAVR) programme] on permanent pacemaker implantation (PMI) after isolated surgical aortic valve replacement (SAVR) in patients with bicuspid aortic valve (BAV) and tricuspid aortic valve (TAV). A retrospective single-center analysis was performed on 4,154 patients who received isolated SAVRs (w/o concomitant procedures), between 2000 and 2019; of these, 1,108 had BAV (27%). BAV patients vs their TAV peers were on average 10 years younger (59.7 ± 12 vs 69.3 ± 9) and had less comorbidities at the time of SAVR. With SAVR, BAV patients seemingly had significantly higher overall incidence of PMI; however, after propensity matching, there appeared no difference in PMI rates between BAV and TAV. Remarkably lower PMI rate was observed among BAV patients after SAVR compared with the reported incidence after TAVR.
