Sugammadex vs neostigmine after rocuronium continuous infusion in patients undergoing liver transplantation

Deana C, et al. - Researchers conducted this randomized controlled trial among patients undergoing orthotopic liver transplantation, to evaluate the neuromuscular transmission recovery time attained with sugammadex vs neostigmine following rocuronium induced neuromuscular blockade in these patients. Intraoperative neuromuscular block was maintained with a continuous infusion of rocuronium. Standardized anaesthetic management, as per institution’s internal protocol, was provided. By administering 2 mg/kg of sugammadex or 50 mcg/kg of neostigmine (plus 10 mcg/kg of atropine), experts obtained neuromuscular moderate block reversal at the end of surgery. They studied the data from 41 patients. According to the findings, the reversal of neuromuscular block maintained by rocuronium continuous infusion was enabled by sugammadex administration in patients undergoing liver transplantation. A significantly faster mean reversal time was achieved with sugammadex vs neostigmine. In this population vs other surgical settings, the observed sugammadex recovery time was considerably longer, and should be considered in clinical practice.
