The influence of time to adrenaline administration in the PARAMEDIC2 randomised controlled trial

Perkins GD, Kenna C, Ji C, et al. - Among participants in the Pre-Hospital Assessment of the Role of Adrenaline: Measuring the Effectiveness of Drug Administration in Cardiac Arrest (PARAMEDIC2) randomised controlled trial performed across 5 NHS ambulance services in England and Wales, researchers assessed the time to drug administration in patients with a witnessed cardiac arrest. Random allocation was done of patients with an out-of-hospital cardiac arrest who were unresponsive to initial resuscitation efforts, to receive 1 mg intravenous adrenaline or matching placebo as per treatment packs that were identical apart from treatment number. Findings revealed a reduction in the rate of return of spontaneous circulation, survival and favourable neurological outcomes over time. It was noted that as time to drug treatment progress, adrenaline raises the likelihood of return of spontaneous circulation. The time to adrenaline administration had no influence on longer term results.
