Benefit of intravenous thrombolysis in acute ischemic stroke patients with high cerebral microbleed burden

Schlemm L, et al. - In individuals with acute ischemic stroke and a high burden (> 10) of cerebral microbleeds (CMBs), researchers estimated the treatment outcome of intravenous thrombolysis (IVT). Among patients with ≤ 10 compared with > 10 CMBs who do or do not receive IVT, they devised a multistep algorithm to model 90-day modified Rankin Scale scores. In probabilistic sensitivity analyses, uncertainty was quantified. The data presented in this work showed a high CMB burden affects the impact of IVT therapy. IVT is associated with higher mortality in patients with > 10 CMBs and a net loss of utility in older patients with severe strokes and longer delays in treatment. If it does not increase the treatment time, patients with higher-than-average pretest probability of > 10 CMB could benefit from MRI screening.
