Limited role of DWI with apparent diffusion coefficient mapping in breast lesions presenting as non-mass enhancement on dynamic contrast-enhanced MRI

Avendano D, Marino MA, Leithner D, et al. - In this retrospective study involving 95 individuals, researchers evaluated the diagnostic precision of diffusion-weighted imaging (DWI) using a different region of interest (ROI) measurement approaches and apparent diffusion coefficient (ADC) metrics in breast lesions presenting as non-mass enhancement (NME) lesions on dynamic contrast-enhanced (DCE) MRI. The sample consisted of individuals who had multiparametric MRI with DCE and DWI from September 2007 to July 2013 and who were diagnosed with a suspicious NME (BI-RADS 4/5). In NME lesions, DWI's diagnostic accuracy with ADC mapping is limited. Data reported that 31% of lesions presenting as NME on DCE-MRI could not be assessed with DWI, and therefore, DCE-MRI remains indispensable. Using darkest part of the tumor 2D ROI measurement and ADC mean, best outcomes were obtained.
