Effect of intra-amniotic fluid pressure from polyhydramnios on cervical length in patients with twin-twin transfusion syndrome undergoing fetoscopic laser surgery

Bergh EP, et al. - Researchers examined how intra-amniotic pressure is correlated with cervical length (CL) in patients with twin-twin transfusion syndrome (TTTS) undergoing fetoscopic laser photocoagulation (FLP). Further, they sought for pre- or intraoperative factors connected with increased intra-amniotic pressure in this population. They conducted a prospective cohort study of 283 pregnancies undergoing FLP for TTTS. Quintero stage of TTTS was I, II, III, and IV in 33, 88, 150, and 12 pregnancies, respectively. The analysis revealed no correlation of intra-amniotic pressure with CL or gestational age at delivery in pregnancies with TTTS. This novel outcome indicates that in this population, cervical shortening is not mechanically driven. Intra-amniotic pressure was noted to be significantly correlated with the following factors: the number of prior term deliveries, recipient maximum vertical pocket, Quintero stage IV and type of anesthesia (sedation vs general anesthesia).
