Management of osteoporosis in patients living with HIV—A systematic review and meta-analysis

Starup-Linde J, et al. - Via performing a systematic review and meta-analysis, researchers assessed evidence on fracture risk in patients living with HIV (PLHIV). In addition, they compared bone mineral density (BMD) in PLHIV vs controls, investigated the longitudinal changes in BMD in PLHIV, and evaluated the effect of antiosteoporosis treatment in PLHIV. The systematic review included 142 papers and the meta-analysis included 84 papers. Outcomes revealed increased risks of a fragility fracture and hip fracture in PLHIV. Relative to controls, PLHIV exhibited lower BMD at the hip and lumbar spine. The increased fracture risk in PLHIV could not be fully explained with the reduced BMD. Overall, the investigators concluded that PLHIV with higher BMD may benefit from anti-osteoporosis intervention.
