The prognostic value of serum procalcitonin measurements in critically injured patients: A systematic review

AlRawahi AN, AlHinai FA, Doig CJ, et al. - By analyzing 19 eligible studies (17 prospective; 2 retrospective) identified from PubMed, MEDLINE, EMBASE, the Cochrane Database, and references of included articles, researchers assessed the prognostic significance of serum procalcitonin (PCT) levels following trauma as associated with severity of injury, sepsis, organ dysfunction, and mortality. A strong link between initial PCT levels and Injury Severity Score was revealed in all studies examined. Patients who later developed sepsis following trauma had significant rise of initial PCT levels, as revealed in 12 out of 16 studies. Seven out of nine studies showed that the PCT level seemed a strong predictor of multiple organ dysfunction (MOD). The link of the PCT level of ≥ 5 ng/mL with significantly increased mortality was shown in one study. The seemingly promising utility of PCT as a surrogate biomarker for trauma was suggested in this study. The possible use of initial peak PCT level as an early predictor of sepsis, MOD, and death in the trauma population was also suggested.
