Brescia G, et al. - Researchers retrospectively evaluated a consecutive series of 17 adult patients, to newly determine the prognostic role (in terms of recurrence rate) of preoperative blood eosinophil as well as basophil levels in allergic fungal rhinosinusitis (AFRS). The participants having AFRS had underwent endoscopic sinus surgery. Among 17 patients, the recurrence of sinonasal polyps was found in 7. Taking the entire cohort into account, they identified a significant positive correlation between blood eosinophil and basophil counts, but not between blood and tissue eosinophil counts. In statistical analysis, they noted that AFRS patients who relapsed had significantly higher blood eosinophil and basophil levels as compared with those who did not. In further prospective and larger (preferably multi-institutional) explorations, testing of blood eosinophil and basophil levels as part of the preoperative work-up for patients with AFRS, so that dedicated postoperative medical treatments may be administered for patients at higher risk of relapse, was supported by this preliminary data, taking into account the current problem of finding more effective, personalized approaches to postoperative disease management in AFRS.
