Development and validation of a pragmatic electronic phenotype for CKD

Norton JM, Ali K, Jurkovitz CT, et al. - In order to recognize patients likely to have CKD, researchers sought to develop and validate a pragmatic electronic (e-) phenotype in this study. An expert working group constructed the e-phenotype. Among adults receiving in- or outpatient care at 5 healthcare organizations, this e-phenotype was implemented. Most recent eGFR < 60 ml/min per 1.73 m2 with at least one value < 60 ml/min per 1.73 m2 > 90 days prior and/or a urine albumin-to-creatinine ratio of ≥ 30 mg/g in the most recent test with at least one positive value > 90 days prior defined the CKD e-phenotype. The CKD e-phenotype had a sensitivity of 99%, specificity of 99%, and diagnostic accuracy was 98%. The reported sensitivity and specificity for dialysis were 94% and 89%, respectively, and for transplant, these values were 97% and 91%, respectively. For EHR-based identification of patients likely to have CKD, a pragmatic and accurate method is offered by CKD e-phenotype.
