A comparison of clinical outcomes of acute testicular torsion between prepubertal and postpubertal males and the effect of age and presenting symptoms on outcome

Goetz J, Roewe R, Doolittle J, et al. - Through a retrospective chart review carried out on 91 pediatric patients who were diagnosed with acute testicular torsion between June 2010 and August 2017, researchers hypothesized that there was a variation in orchiectomy and orchiopexy rates among prepubertal and postpubertal males with acute testicular torsion and presenting symptoms may differ between the two age groups as prepubertal males may present with atypical symptoms, which could lead to postponed presentation and diagnosis. In comparison with postpubertal males, more prepubertal males underwent orchiectomy. Prepubertal males were more probable to present with abdominal pain when compared with postpubertal males. On the contrary to those who underwent orchiopexy, those who underwent orchiectomy were more expected to present with a more prolonged-term of symptoms, testicular swelling, and abdominal pain. From 1 to 12 years of age, a constant drop in the proportion of patients who underwent orchiectomy was observed. In conclusion, prepubertal males are at greater risk for orchiectomy as that of postpubertal males. The risk of orchiectomy reduced by 14%–16% per 1-year increase in age. Prepubertal males were more inclined to present with atypical symptoms which led to delayed presentation and diagnosis, resulting in a delay in surgical intervention. Therefore, in prepubertal males who present with abdominal pain, it was necessary for providers to conduct a genital examination to rule out acute testicular torsion. Individuals who presented with longer duration of symptoms, testicular swelling, and abdominal pain are at higher risk for orchiectomy. Moreover, no association was discovered between orchiectomy rate and ethnicity, referral status, primary insurance status, and time/weekday/season at presentation.
