Concordance of cribriform architecture in matched prostate cancer biopsy and radical prostatectomy specimens

Hollemans E, Verhoef EI, Bangma CH, et al. - A total of 186 matched prostate biopsies and radical prostatectomy specimens were assessed by the researchers in order to discover the extent of cribriform architecture undersampling and to find predictive factors for recognizing false cribriform-negative cases. Twenty-two, 39 and 36 had true cribriform-negative (TN), false-negative (FN) and true-positive (TP) biopsies, respectively, of 97 biopsy grade group 2 individuals. However, not statistically notable, but individuals with FN biopsies had greater median prostate-specific antigen (PSA) levels vs individuals with TP biopsies. In nine of 16 FN and three of 11 TN biopsies, a Prostate Imaging Reporting and Data System (PI-RADS) 3-5 lesion was found. No variation in positive biopsy rate, percentage Gleason pattern 4 and glomeruloid architecture was noted. For cribriform architecture on radical prostatectomy, logistic regression classified PSA as an independent prognosticator and not PI-RADS score. For large cribriform architecture at radical prostatectomy, the FN rate was 27%, which was lesser than for any cribriform architecture. In individuals with TP biopsies, biochemical recurrence-free survival was significantly shorter than in comparison with those with FN biopsies, during follow-up. Hence, for cribriform architecture, 40% of grade group 2 prostate cancer biopsies were FN. Moreover, these individuals had greater PSA levels and more frequent PI-RADS score 5 lesions vs men with TN biopsies.
