Joint effects of advancing age and number of potentially inappropriate medication classes on risk of falls in Medicare enrollees

Early NK, et al. - Using data from the Truven Health MarketScan Medicare database (2013–2015), researchers investigated the impact of advancing age and of use of fall-risk drugs/potentially inappropriate medications (FRD/PIM) on the risk of falls among Medicare enrollees. They used a case-control design and compared 110,625 enrollees aged 65–99 years diagnosed with > 1 fall event to 1,567,412 enrollees without falls. Based on number needed to harm (NNH) calculations for each FRD/PIM therapy class count (vs 0 FRD/PIMs), stratified by age group, they assessed the joint age-FRD/PIM impacts on fall risks in an exploratory analysis. Findings are indicative of joint effects of FRD/PIM use and age on fall risk. The possible suitability of older adults at high risk, indicated by small NNH, for fall prevention initiatives was suggested and clinicians may want to consider reducing the number of FRD/PIMs utilized by these patients.
