Septal fractures predict poor outcomes after closed nasal reduction: Retrospective review and survey

Arnold MA, et al. - In this retrospective review and survey, researchers determined outcomes of patients with displaced nasal bone fractures following closed nasal reduction (CNR). The study sample consisted of 607 patients presented to the emergency department in 2015 and 2016 with a diagnosis of nasal bone fracture. Of these, 134 patients met the criteria for inclusion and had CNR without septal reduction. Findings revealed that fractures of the nasal bone are a common injury, often initially managed with CNR. The elevated risk of posttraumatic nasal deformity and obstruction despite CNR should be advised to patients with septal fractures. Furthermore, addressing a septal fracture found on the image may be warranted with either a closed septal reduction or early aggressive management due to the poorer results seen in the present study. While these patients are more likely to receive definitive therapy, many forego subsequent intervention despite persistent symptoms, highlighting the need for early intervention or close follow-up.
