Assessment of the rate of skip metastasis to neck level IV in patients with clinically node-negative neck oral cavity squamous cell carcinoma: A systematic review and meta-analysis

Warshavsky A, et al. - Researchers performed a study using meta-analysis in patients with clinically node-negative neck (cN0) oral cavity squamous cell carcinoma (OCSCC) in order to determine the rate of skip metastasis to neck level IV with a high level of evidence. In patients with cN0 OCSCC, the rate of skip metastasis to neck level IV was very low. Data reported that the overall rate of level IV involvement was between 0% and 11.4%. In cases including neck levels I through III, rate of level IV skip metastasis rate did not rise significantly. During neck dissection, finding an allegedly positive lymph node did not imply high rates of level IV involvement. Therefore, supraomohyoid neck dissection seems to be a satisfactory treatment for patients with cN0 OCSCC.
