Secondary surgical resection for patients with recurrent uterine leiomyosarcoma

Cybulska P, et al. - In this retrospective analysis, researchers evaluated results post-secondary surgical resection in patients with recurrent uterine leiomyosarcoma (uLMS). Overall 62 patients with no evidence of disease following initial surgery for uLMS, and who had operation for a first recurrence, were identified. Of these, 29 had abdominal/pelvic recurrence only, 30 had lung recurrence only, 3 had both. For abdominal/pelvic recurrence and for lung recurrence, the estimated median overall survival (OS) was 37.7 months and 78.1 months, respectively. In 58 cases (93%), complete gross resection (CGR) was achieved, with gross residual ≤1 cm in 2 (3.5%) and >1 cm in 2 (3.5%). Based on residual disease, 54.1 months, 38.7 months, 1.7 months, respectively, were the median OS. In patients with a high chance of achieving CGR, it is reasonable to go for secondary surgical resection of recurrent uLMS. More favorable outcome was reported in relation to lung-only recurrences. Additional therapy may not be beneficial after CGR.
