General anesthesia vs conscious sedation for intracranial mechanical thrombectomy: A systematic review and meta-analysis of randomized clinical trials

Journal of the American Heart Association Jun 17, 2019

Zhang Y, et al. - In order to determine the impact of anesthetic strategy during endovascular treatment for acute ischemic stroke, researchers analyzed randomized clinical trials of relevance in this systematic review and meta-analysis performed following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The included studies were identified from Medline, EMBASE, and CENTRAL databases. As per moderate-quality evidence gained in this study, significantly higher rates of functional independence were reported in relation to the use of general anesthesia vs conscious sedation in patients with ischemic stroke undergoing endovascular therapy. However, a higher risk of 20% mean arterial pressure reduction was observed in relation to general anesthesia. With regard to mortality, symptomatic intracranial hemorrhage, anesthesiologic complication, intensive care unit length of stay, pneumonia, and interventional complication, no significant differences were evident.
