Gleason grade patterns in nodal metastasis and corresponding prostatectomy specimens: Impact on patient outcome

Downes MR, et al. - Via a retrospective study of 110 cases of positive prostatectomies, the researchers sought to explore the correlation of the pattern of lymph nodal metastases at the time of the prostatectomy with patient outcome. Documentation of the presence of cribriform carcinoma (CC), intraductal carcinoma (IDC) and ISUP grade (G) was observed. The largest nodal metastasis was checked for morphologic patterns present. Based on percentage morphologic patterns present G was assigned to the metastasis. No prognostic correlation was shown by nodal cribriform pattern and a significant trend towards better outcome was portrayed by pattern 3 metastasis. Identification of IDC or CC in 94% of node positive prostate cancers were concluded by the authors. Although, prognostic significance of G in the largest nodal metastasis was noted, it could not be reflected in that of the primary prostatic adenocarcinoma.
