Fuchs-Buder T, et al. - Researchers conducted a single-center, randomized controlled study investigating the influence of switching from moderate to deep neuromuscular block (NMB) on surgical conditions for laparoscopic surgery in the obese. In addition, they determined changes in intra-abdominal pressure, the time required to perform the gastrojejunal anastomosis and peri-operative surgical complications. They examined 89 patients who underwent laparoscopic gastric bypass surgery under general anesthesia, twice: at the first evaluation, all patients had a moderate NMB, thereafter patients were randomized to deep or moderate block and a second evaluation was performed within 10 min. In this study, each patient was taken as their own control. Outcomes revealed improvement in surgical conditions when switching from moderate to deep block was done. They noted a higher incidence of surgical complications to be related to poor surgical conditions.
