Blood pressure and incident atrial fibrillation in older patients initiating hemodialysis

Chang TIH, et al. - In older patients (aged ≥67 years) initiating hemodialysis, researchers assessed the relation of predialysis systolic and diastolic BP and intradialytic hypotension, to incident atrial fibrillation by using the US Renal Data System linked to the records of a large dialysis provider. Atrial fibrillation developed in 3785 of 17,003 cases. A higher hazard of atrial fibrillation was reported in relation to lower predialysis systolic BP, when predialysis systolic BP was compared to a fixed reference of 140 mm Hg. A lower hazard of atrial fibrillation was reported in relation to higher systolic BP. They found a link between lower diastolic BP and higher atrial fibrillation hazards. A weak association of intradialytic hypotension with atrial fibrillation was also observed. Overall, higher incidence of atrial fibrillation was reported in relation to lower predialysis systolic BP and diastolic BP in this observational study of older patients initiating hemodialysis.
