Effect of high PEEP vs low PEEP on postoperative pulmonary complications in obese patients: A randomized clinical trial

Bluth T, et al. - Through a randomized clinical trial carried out on 1,976 obese adults undergoing non-cardiac, non-neurological surgery, the researchers sought to understand the reduction in postoperative pulmonary complications when compared among the lower level of positive end-expiratory pressure (PEEP) and a higher level PEEP with alveolar recruitment maneuvers. A higher level of PEEP and alveolar recruitment maneuvers in comparison to a strategy with a lower level of PEEP as an intraoperative mechanical ventilation strategy, among obese patients undergoing surgery under general anesthesia, no significant decrease in postoperative pulmonary complications were noticed. High level of PEEP and alveolar recruitment maneuvers having adverse effects were concluded.
