Partial vs radical nephrectomy in very elderly patients: A propensity score analysis of surgical, functional and oncologic outcomes (RESURGE project)

Mir MC, et al. - Researchers studied a purpose-built multi-institutional international database (RESURGE project) to analyze the consequences of partial nephrectomy (PN) vs radical nephrectomy (RN) in very elder subjects treated for clinically localized renal tumor. They recorded a higher overall complication rate in the PN group (33% vs 25%). A significant difference between RN and PN in a median decline of eGFR was also reported. They did not observe an association between PN and overall mortality (OM), other-cause mortality (OCM). Whereas PN revealed a protective inclination for cancer-specific mortality (CSM), surgical CKD and worsening of eGFR in patients with baseline CKD. They did not find a compromise in oncological outcomes when PN was adopted in very elderly subjects with localized renal tumor which allowed better functional preservation at mid-term (3-year) follow-up when compared to RN.
