The impact of a community-based serious illness care program on healthcare utilization and patient care experience

Daaleman TP, et al. - Researchers assessed healthcare utilization and the patient care experience among 159 patients enrolled in the REACH program, which was developed as a primary and palliative care program for seriously ill older adults. An interdisciplinary team provided home-based primary and palliative care to study participants. A 43% attenuation in hospital visits and a 25% reduction in emergency department (ED) visits was reported among REACH patients. As reported by participants during telephone interviews, high-quality care and very good quality of communication were provided by REACH providers. Therefore, reduction in health service use and enhancement in the care could potentially be achieved with a care model that integrates home-based primary care and palliative care for older patients with serious illness.
