An analysis of the impact of fluid overload and fluid depletion for all-cause and cardiovascular mortality

Siriopol D, et al. - Performing a retrospective cohort study with 4,114 hemodialysis patients from 34 Romanian dialysis units, researchers evaluated all-cause and cardiovascular mortality risk in relation to baseline pre- and post-dialysis volume status, changes in pre- and post-dialysis fluid status during follow-up (time-varying survival analysis), pre–post changes in volume status during dialysis, and pre-dialysis fluid status variability in the first 6 months of evaluation. Findings revealed a significantly elevated risk of all-cause mortality in relation to baseline pre-dialysis fluid overload (FO) and fluid depletion patients. In contrast, better survival was evident in association with post-dialysis fluid depletion. During follow-up, they also noted FO patients had an increased risk of all-cause and cardiovascular mortality as compared with normohydrated patients. Patients with high-amplitude fluctuation during the first 6 months of evaluation displayed the highest risk for the all-cause mortality outcome. Findings thereby reconfirm that mortality in dialysis patients was linked with baseline pre- and post-dialysis volume status.
