Risks and benefits of direct oral anticoagulants across the spectrum of GFR among incident and prevalent patients with atrial fibrillation

Shin JI, et al. - to warfarin in terms of bleeding risk and benefit of ischemic stroke prevention via Cox proportional hazards regression. In an electronic health record (Geisinger Health System), they created a propensity score-matched cohort of 3,206 patients using direct oral anticoagulants and 3,206 patients using warfarin from October of 2010 to February of 2017. They found that, relative to warfarin, treatment with direct oral anticoagulants for atrial fibrillation in patients with eGFR < 60 ml/min per 1.73 m2 resulted in slightly higher risk of bleeding, but similar benefits for the prevention of ischemic stroke.
