Anesthetic management of women with heart failure during pregnancy - A retrospective cohort study

Jayasooriya G, Silversides C, Raghavan G, et al. - Via retrospectively studying a cohort of women presenting with heart failure in pregnancy between 2004 and 2017, researchers sought to inscribe the management and etiology of heart failure among these women. From 2004 to 2017, they identified 120 cases (in 93,122 deliveries; antepartum heart failure 51%, postpartum heart failure 49%. This suggests the heart failure incidence of 0.13% (1:776 deliveries). The leading etiological factor was preeclampsia but it may have been historically under-acknowledged. They identified reporting of half of the cases of heart failure immediately postpartum. By means of multidisciplinary input, anesthetic and obstetrical decisions were tailored, with majority being managed with cesarean delivery and regional anesthesia. Findings support following up of these patients more intensely during the postpartum period.
