Transperitoneal vs retroperitoneal robotic partial nephrectomy: A matched-paired analysis

Mittakanti HR, et al. - A total of 281 patients who underwent retroperitoneal partial nephrectomy (RPPN) and 263 patients who underwent transperitoneal partial nephrectomy (TPPN) were recruited in a matched-paired analysis by the experts in order to discuss the technique for robotic RPPN and contrast both RPPN and TPPN at a single institution. In the RPPN group vs TPPN group, operative time and predicted blood loss were significantly less. Relative to complexity of cases, warm ischemia time, tumor pathology, positive margin rates, complications, or kidney function postoperatively, no variations were observed. Hence, robotic RPPN and TPPN could both be utilized for nephron-sparing surgery with good outcomes. RPPN, when used correctly, could result in shorter operative times, less blood loss and equivalent oncologic and postoperative outcomes. Surgeon comfort and expertise would aid to ascertain which strategy to use.
