Surveillance factors change outcomes in patients with hepatocellular carcinoma due to chronic hepatitis C virus infection in New Zealand

Schauer C, et al. - A total of 520 individuals with confirmed hepatocellular carcinoma (HCC) secondary to chronic hepatitis C virus (HCV) were identified from a prospective national HCC database by the experts in order to review cases of HCC secondary to HCV to determine the influence of adherence with HCC surveillance on survival. In 224 individuals, HCC was detected. HCC was discovered either accidentally or after the onset of symptoms in nonadherent, suboptimal surveyed, undiagnosed cirrhotic or recently diagnosed HCV individuals or were never offered surveillance. The outcome following a diagnosis of HCC secondary to chronic HCV was ascertained by early detection when the curative intervention was possible. Lack of diagnosis of HCV and nonadherence to HCC surveillance led to late diagnosis and poor outcomes. Moreover, in individuals known to have HCV infection, under-diagnosis of HCV infection and lack of diagnosis of cirrhosis decreased the advantage of current HCC surveillance approaches.
