Comparison of the sepsis-2 and sepsis-3 definitions in severely injured trauma patients

Eriksson J, Eriksson M, Brattström O, et al. - In this single-centre, retrospective, observational study, researchers assessed the performance of the new SOFA-based sepsis definition in trauma patients. Thirty-day mortality including a censoring analysis for early deaths was the primary outcome, which was assessed using logistic regression, receiver operating characteristics curves and Kaplan-Meier survival analyses. Overall 722 severely injured patients were analyzed. The sepsis-2 criteria and the sepsis-3 criteria were met by 315 and 148 patients, respectively, during the first 10 days in the ICU. For sepsis-2 and for sepsis-3, the estimated odds ratios for 30-day mortality were 0.7 and 1.5, respectively. Much fewer patients were identified using the sepsis-3 definition. Compared with the sepsis-2 definition, a more strong association with adverse outcomes was evident for the sepsis-3 definition. Findings are suggestive of the utility of the sepsis-3 definition in the posttrauma setting.
