Clinical significance of genotyping for human papillomavirus (HPV) 16 18/45 combined with cytology in cervical exfoliated cells in HPV oncogenic mRNA-positive women

Wang J, et al. - In women who screened positive in a human papilloma virus (HPV) E6/E7 mRNA assay (Aptima HPV, AHPV), researchers evaluated HPV 16 18/45 typing test outcomes in combination with cytology for cervical exfoliated cells. Of 3,257 AHPV-positive women aged 25–65 years, 624 (19.2%) tested AHPV-GT (AHPV 16 18/45 Genotype assay) positive. A higher specificity and positive predictive value (PPV) were offered by AHPV-GT vs cytology when identifying CIN2+, with a similar AUC. A higher sensitivity, higher NPV and similar PPV was offered by the co-testing strategy (cytology with AHPV-GT) vs cytology when identifying CIN2+, with an increase in AUC from 0.69 to 0.76. In women receiving AHPV-positive primary screening tests, high specificity was offered by AHPV-GT test and it could be a capable triage approach. Although the same CIN2+ and CIN3+ cases were identified with a combination strategy and co-testing strategy, significantly fewer screening tests were needed with the former approach.
